Because I am not able to create a valid nor secure form (please read my help message below) I am currently using 1 of 2 forms. The first order form is in MS Word. The second order form is a PDF document. Please click on the link you wish to use to download. You can then print and mail the form to me here in Japan. Please leave 7 to 10 days if you airmail me and leave up to 90 days if you send your order by seamail (though it usually takes about 1 month). Also while you can send cash you do so at your own risk — letters do get lost in the mail! I cannot be responsible for letters I do not receive. Please make all checks or money orders payable in US dollars.

Help!

I am using Adobe GoLive to create this site and am using Adobe Creative Suite for Dummies as a guide on how to make webpages. The guide is 50% useless! As to how to create a secure form, it has the helpful words, "This is outside the scope of this chapter," and that's it. No where in the guide do they tell how to make one. Great hey? Also following the guide's instruction I cannot even get the form to be sent to me. Right now all that happens is you type in all the blanks, hit send and get a blank email page! So as of now, I have deleted the internet form. If anybody knows how to create a secure form or a good, valid guide to use, please email me.
